Eligibility & Certification

Physical Examination:
Proof of a student-athlete's physical exam must be submitted to the school every 365 days in order for the athlete to be eligible.

For example: A student-athletes' physical was on March 5, 2003. That physical is OK for eligibility until October 5, 2009 . On October 5, 2010 a new physical form must be submitted. Parents need to realize the physical expiration date could come mid-season which means new proof of a physical needs to be submitted to the school mid-season. Parents should should contact their doctor to find out the last date of a physical. This will leave plenty of time for a doctor's appointment if needed.

Parental Permission:
Each student-athlete must submit the signed parental permission card once an academic year.  The form can be found under the News and Announcements section of the SHS home page.


Activity Fee:
There is a $300.00 activity fee per sport for each athlete with a family cap of $900 that must be paid once an academic year for all students that participate in athletics. This should be paid with a check payable to SHS.

Emergency Information Sheet:
This form must be filled out by the parent and returned before an athlete will be certified. This sheet contains pertinent information about the student-athlete. It includes medical allergies, emergency contact information, and previous injuries. All coaches will keep these sheets on hand for practices and games.   The forms can be found under the News and Announcement section of the SHS home page.


Athletic Handbook:
This reviews all athletic rules.  Each student-athlete and parent/guardian must sign the back page of this handbook and return before certification.

Academic Eligibility:
Each student-athlete must secure a grade of C- or above in five courses during the term preceding the season. If this is not met, the student-athlete will be ineligible until the next report card shows eligible grades.


Massachusetts Anti-Hazing Information:

Each institution of secondary education and each public and private institution of post secondary education shall issue to every student group, student team or student organization which is part of such institution or is recognized by the institution or permitted by the institution to use its name or facilities or is known by the institution to exist as an unaffiliated student group, student team or student organization, a copy of An Act of Prohibiting the Practice of Hazing. Coaches will distribute a copy of this to each member of his or her team. Participants will agree to comply with the law.

Each such group, team or organization, acting through its designated officer, to deliver annually, to the institution an attested acknowledgement stating that such group, team or organization has received a copy of this section. Coaches will complete the following Annual Statement of Acknowledgement for Student Groups, Teams, and Organizations and submit it to the Principal by October 1, 2010.
